Job Description: Assisting a pleasant, elderly woman in her private, well kept home with the following diagnosis: Alzheimers, Hypertension, End Stage Renal Disease, Permacath for dialysis Client Dependent with dressing, toileting, PM care, ambulation (wheelchair/walker), transfers with assist and can ambulate short distances with assist X 1, needs assist with telephone, in home activities such as (games/puzzles), conversation, cooking and serving meals, light housekeeping, transportation to and from dialysis on Tuesdays and Saturdays (drop off at 10am and pick up at 2:00pm, client has sitter in dialysis so need to stay), medication administration (will have prepared pill box), some physical therapy, and fall precautions (client cannot be left alone). Family will be responsible for groceries/grocery shopping, MD appointments, preparing pill box/medication management, showering, hair/nail care, home repairs, lawn care and finances. Other Services: Provided Nurse visits once weekly, Physical therapist twice weekly, Nurses Aide twice weekly. Hap at Home pre Hospice program. Hospice volunteers (call ahead to set up volunteer if you need to be out of the home). Neighbors, friends and family (list of phone numbers to be provided).
